Position Overview
BrightStar Care of Omaha is hiring an experienced Registered Nurse (RN) Case Manager with strong in-home infusion experience. This role is designed for an RN who excels in clinical leadership, infusion therapy, and care coordination in the home setting.
This position supports BrightStar Care of Omaha's commitment to high-quality, patient-centered care and compliance with Joint Commission accreditation standards.
Key Responsibilities
- Provide skilled in-home infusion therapy per physician orders and care plans
- Serve as RN Case Manager overseeing infusion patients from admission through discharge
- Conduct comprehensive nursing assessments and ongoing clinical evaluations
- Administer IV medications, manage PICC lines, ports, and central lines
- Educate patients and caregivers on infusion therapy, medications, equipment, and infection prevention
- Coordinate care with physicians, specialty pharmacies, and interdisciplinary teams
- Ensure accurate, timely documentation in compliance with regulatory standards
- Follow infection control, patient safety, and quality improvement standards
- Participate in on-call rotation as needed
- Support quality assurance and performance improvement initiatives
Required Qualifications
- Active Registered Nurse (RN) license in the state of Nebraska
- Strong in-home infusion experience
- Experience with IV therapy, central lines, PICC lines, and ports
- Excellent clinical judgment, communication, and organizational skills
- Ability to work independently in a home care environment
- Valid driver's license and reliable transportation
Preferred Qualifications
- Prior RN Case Management experience
- Strong in-home infusion experience (preffered but not required)
- Home health or private duty nursing background
- Experience coordinating care with specialty infusion pharmacies
- Familiarity with electronic medical record systems
- Organized, autonomous, and excellent communication skills
Compensation & Benefits
- Pay range: $35–$45 per hour, based on experience
- 36–40 hours per week
